PARENT PARTICIPATION AGREEMENT

I, ______________________________ as parent of ________________________ agree to the following:

  • will include post-dated cheques or a credit card imprint for parent participation with my child’s registration form and tuition fees;
  • post-dated cheques or credit card imprints will be in the amount of $225.00 each and dated January 15, 2007 and June 15, 2007;
  • will provide 15 hours of participation between September and December;
  • will provide 15 hours of participation between January and June;
  • if unable to fulfill my parent participation hours, I understand that I will be billed at a rate of $15.00 per hour to a maximum of 15 hours per period and this amount will be recovered by cashing my parent participation cheque for the period, the difference to be paid back to me by a PAC cheque;
  • if I have committed to an event and find myself unable to assist, I am responsible for finding a suitable replacement (understanding that children may not substitute for their parents);
  • upon completion of an event, I will record my hours of parent participation in the binder located at the entrance to the staff room;
  • will review my statements to ensure accuracy prior to parent participation cheques being cashed.

The PSG and James Cameron School are responsible to:

  • provide periodic updates of hours by The Parent Participation Coordinator;
  • send home notices asking for volunteers for driving to events such as  library, swimming and field trips; and
  • provide statements indicating hours volunteered in each period, to be provided in January and June prior to the parent participation cheques being cashed.
